With the Glymer chrome extension, you can create annotations whenever you encounter mis-information on a website. Just highlight the text you want to annotate, right click, and select "Annotate with Glymer". Not only can you create annotations, but the extension comes with a sidebar that shows annotations from other users on the web pages you visit.
